Abstract
Hypertension remains a significant public health concern in Indonesia due to its contribution to morbidity and mortality. Beyond its physiological consequences, hypertension may also be accompanied by psychological conditions, such as depression, which can influence patients’ motivation and ability to adhere to long-term medication therapy. Aim. This study aimed to investigate the relationship between depression levels and medication adherence among hypertensive patients receiving treatment at community health centers in Makassar City. Methods. A cross-sectional approach was applied involving hypertensive patients treated at four community health centers in Makassar City from March to May 2024. Depression severity was assessed using the Beck Depression Inventory-II (BDI-II), while medication adherence was evaluated using the Medication Adherence Rating Scale (MARS). The association between depression and medication adherence was examined using multivariate logistic regression analysis. Results. A total of 185 hypertensive patients were included in the study. The findings showed that 71.4% of participants had moderate medication adherence, whereas 28.6% demonstrated high adherence. Based on BDI-II assessment, 33% of patients were categorized as normal, 55.1% experienced mild mood disorders, 6.5% had borderline depression, and 5.4% experienced low depression. Bivariate analysis showed no significant association between patient characteristics and medication adherence. However, multivariate analysis revealed a significant relationship between depression level and medication adherence (OR = 0.420; 95% CI: 0.217–0.814; p = 0.001). Patients with higher depression levels tended to have lower medication adherence. Conclusion. Depression level is associated with medication adherence among hypertensive patients treated at community health centers in Makassar City. Further research involving broader clinical variables and intervention-based approaches is needed to clarify this relationship and support improvements in depression management and medication adherence.
