Abstract
The assessment of compounded prescriptions for pediatric patients is a crucial component of pharmacy services aimed at preventing medication errors. This study aimed to determine the profile of pediatric prescriptions screening, focusing on administrative aspects, limited pharmaceutical parameters (dosage form and strength), and selected clinical aspects (dosage accuracy and drug interactions). It was a retrospective observational study with a cross-sectional design conducted at a community pharmacy in Denpasar, Bali, during November and December 2024. A total sampling technique was used to select 117 pediatric compounded prescriptions. Dosage assessment was based on PIONAS for children and ISO 2019, and drug interactions were analyzed using Drugs.com and the Medscape Drug Interaction Checker. The results showed administratives completeness of 67.9%, pharmaceutical requirements of 45.3%, and clinical requirements compliance of 70.4%. Clinical evaluation revealed dosage discrepancies in 5.1% (6 out of 117 presciptions) and potential drug interaction in 9.4% (11 out of 117 prescriptions). Antibiotics were found in 20.5% of prescriptions (24 out of 117), with the most frequently prescribed being Amoxycillin (41.7%), Cefixime (37.5%), and Azithromycin (20.8%). In conclusion, the completeness of pediatric compounded prescriptions did not yet fully comply with the Indonesian Ministry of Health Regulation No. 73 of 2016. Pharmacists need to enhance their role in prescription screening and collaboration with physicians to ensure patient safety.
