Evaluasi Pengaruh Metformin Terhadap Kadar Testosteron Pada Pasien Pria Dengan Diabetes Melitus Tipe 2 (DMT2): Pre-Posttest Design

Abstract

Diabetes melitus tipe 2 (DMT2) pada pria kerap disertai penurunan kadar testosteron. Metformin merupakan terapi lini pertama yang efektif mengendalikan glukosa darah, tetapi pengaruhnya terhadap hormon testosteron masih menjadi perdebatan. Penelitian ini bertujuan mengevaluasi pengaruh dosis metformin, usia, perubahan indeks massa tubuh (IMT), serta kadar gula darah sewaktu (GDS) terhadap perubahan kadar testosteron pada pasien pria dengan DMT2. Metode yang digunakan adalah desain pre–post dengan pendekatan kuantitatif analitik pada 79 pasien pria yang menjalani terapi metformin. Data yang dikumpulkan meliputi dosis obat, usia, IMT, GDS, dan kadar testosteron total sebelum serta sesudah terapi. Analisis statistik dilakukan menggunakan uji korelasi Pearson, Spearman, Mann-Whitney, dan regresi linear berganda. Hasil menunjukkan adanya perbedaan bermakna kadar testosteron antara kelompok dosis metformin <1500 mg/hari dan ≥1500 mg/hari (p=0,016). Perubahan GDS memiliki hubungan sangat kuat dengan perubahan testosteron (r=0,773; p<0,001), sedangkan perubahan IMT menunjukkan hubungan sedang (ρ=0,458; p<0,001). Analisis regresi mengidentifikasi perubahan GDS sebagai faktor paling dominan, diikuti oleh IMT, sementara usia dan dosis metformin tidak signifikan. Disimpulkan bahwa penurunan GDS berasosiasi dengan penurunan testosteron, yang kemungkinan berkaitan dengan pengaruh metformin terhadap proses steroidogenesis.
